These forms provided as a resource for clubs that do not already have their own. They can help collect important details from players.

Please note:

• These forms do not replace the Scottish Ice Hockey online registration system, which must still be used for all official SIH player registrations.
• Clubs are responsible for ensuring all data collected is handled in accordance with current data protection regulations.
• Scottish Ice Hockey may request to see a signed copy of a player’s registration form if a dispute arises.
