The form below must be completed for ALL Scottish Region Players & Officials and retained by the club as part of their registration records. A signed copy, or equivalent digital version, must be retained and made available to Scottish Ice Hockey (SIH) or Ice Hockey UK upon request.

Please note:

• These forms do not replace the Scottish Ice Hockey online registration system, which must still be used for all official SIH player registrations.
• Clubs are responsible for ensuring all data collected is handled in accordance with current data protection regulations.
• SIH/Ice Hockey UK may request to see a signed copy of a player’s registration form if a dispute arises..
